SUBJECT: : EMPLOYEE SEPARATION - Procedure

SECTION 1: PURPOSE
The
purpose of this procedure is to delineate the process to be followed upon the
termination or resignation of an employee from employment to ensure accurate
position control and employee records.
SECTION 2: RESIGNATION
The
employee shall submit to his/her department a signed resignation notice stating
the effective date and reason for leaving employment.
All
resigning employees shall submit their resignation notice at least 2 weeks
prior to the effective date of separation in order to permit adequate
replacement arrangements and to remain in good standing with the City of Lodi
after leaving.
SECTION 3: UNAUTHORIZED
ABSENCE
An
employee's failure to notify a supervisor of an absence or to arrive at his/her
designated workplace for three (3) consecutive workdays as scheduled may be
considered an unauthorized absence and/or an abandonment of position. Such absence may be processed as an automatic
resignation or be cause for disciplinary action up to and including
termination.
SECTION 4: PROCEDURES
AFTER NOTIFICATION
The
following procedures are applicable to cases of termination and resignation
after initial notification of such action has been received by the affected
department.
A. The responsible department shall submit
all pertinent documentation including resignation notice or termination
documents, and required Personnel Action Form to the Personnel Department.
B. The Employee will then be sent a notice
from the Personnel Department informing him/her of an exit interview date and
time conducted by the Personnel Department.
C. The employee's supervisor will complete
the Property Return Check List prior to the employee's exit interview. The employee must bring this form to his/her
exit interview.
D. The Finance Department will determine
leave balances.
E. Final checks are normally issued on the
next regularly scheduled payday following the effective date of the
separation. Any other arrangements must
be made by the employee through the Finance Department.
