SUBJECT: : EMPLOYEE PERFORMANCE EVALUATION - Procedure

SECTION 1: PURPOSE
The
purpose of this procedure is to describe the steps to be followed in
implementing the City's policy on Employee Performance Evaluations.
SECTION 2: PROCEDURE
A. The first-line supervisor will give the
employee an Overall Rating as well as individual ratings for the areas listed.
A written narrative must be provided to explain ratings above or below
"Satisfactory." The report is
reviewed and signed by the next-in-line supervisor and finally by the
department head.
B. When the report is actually given to
the employee, the employee is to sign the form and the original is returned to
the Personnel Department for review by the Personnel Director and placement in
the employees' personnel file. The
second copy is retained by the department and the final copy is sent to the
employee.
C. The Employee Performance Report must be
completed before each merit increase eligibility date.
