Return application to the City Clerk’s office by delivery to 221 West Pine Street, or by mail to P.O. Box 3006, Lodi, CA 95241-1910, or by fax to (209) 333-6807.  Applications will remain valid for one year after date received.

Some board, commission, and committee members (as designated in the City's Conflict of Interest Code) may be required to complete a Statement of Economic Interest (SEI) filing as required by the Fair Political Practices Commission (FPPC).
