Library Card Applications

Please bring completed applications to the Lodi Public Library for processing.

Anyone may have free use of the library. A library card is required to check out materials. No patron may have more than one card registered in his or her name. Any resident of California over 18 years of age may obtain a library card. To obtain a library card you will need a valid picture I.D. (such as a drivers license) and proof of your current address. Your first card is free. If your card is lost or destroyed, there will be a $3.00 charge for a replacement.

For Those with Children

Children under 18 years of age should have a parent or legal guardian with them when applying for a card. The parent or legal guardian signing for the child is responsible for any materials checked out by the child. Any restriction of a child's library materials is the responsibility of the parent or legal guardian.

The children's librarian is available to give selection guidance. You must have your library card with you to check out books and/or other materials. All checkouts are done by computer which scans the barcode on your library card and links it with the barcode on the item. If your library card is lost, please notify us at once. You are responsible for any items checked out on your card. If you notify us when your card is lost, we can put a stop on it to prevent unauthorized use.
